Flashcards

Primary site for needle thoracostomy decompression
The primary site is the anterior axillary line at the 5th intercostal space, which corresponds to nipple level in men and the inframammary crease in women. It may be located using the 'armpit handshake' — the lower border of the patient's palm placed in the axillary crease lands at the 5th intercostal space.
Shock: When to reduce IV/IO infusion rate to TKO
If BP does not improve after fluid resuscitation, check the ECG monitor for signs of a cardiogenic cause or signs of obstructive shock such as neck vein distention. If these signs are present, reduce the IV/IO infusion rate to TKO (To Keep Open) and monitor vital signs.
Maximum repeat doses of nitroglycerin for adult chest pain
Nitroglycerin may be repeated for a maximum of three doses in adult chest pain patients. The condition for repeat dosing is a systolic blood pressure greater than 100 mmHg.

  • 81 mg
  • 162 mg
  • ✓ 325 mg
  • 500 mg
The June 2025 update to protocol 2-1-b (Chest Pain: Adult) adjusted aspirin dosing from 162 mg to 325 mg for adults without a history of allergic reaction.
A MICT arrives on scene to find a patient in cardiopulmonary arrest. Per the Hawaii MICT Standing Orders abbreviations, what does 'HP-CPR' stand for?
  • Hospital-Performed Cardiopulmonary Resuscitation
  • ✓ High Performance Cardiopulmonary Resuscitation

The Approved Medical Abbreviations section explicitly defines HP-CPR as 'high performance cardiopulmonary resuscitation.' This is a distinct, protocol-recognized term separate from standard CPR, emphasizing the quality and performance standards expected during resuscitation.

Hawaii protocols — FAQ

Are Hawaii's EMS protocols available offline?
Yes. Download Hawaii's protocol set once and every protocol, medication, and hospital is available with no signal — built for basements, rural calls, and dead zones.
Are the protocols specific to my county in Hawaii?
Yes. Hawaii's protocols are scoped by county and region, so every provider sees exactly the set that governs where they respond. You can add more than one if you run in multiple areas.
Is Pocket Protocols official, or affiliated with Hawaii?
No — Pocket Protocols is an independent app and isn't affiliated with or endorsed by any EMS authority. We bring Hawaii's protocols into a faster, fully offline app and link the authority's own source for every set.
How do Hawaii protocol updates reach the app?
When the EMS authority publishes a new version and it goes live in Pocket Protocols, the app refreshes automatically — crews are never working from a stale copy. We monitor official sources for changes every day.
